Full Job Description

Job Overview:

We are seeking a dedicated Nurse Practitioner/Physician Assistant to provide primary care to a high-acuity geriatric population residing in senior living communities. You will serve as the cornerstone in a comprehensive healthcare model that focuses not only on clinical excellence but also on meaningful community engagement and proactive patient education. You’ll be responsible for a spectrum of care—from preventive and routine care to urgent clinical interventions—all while fostering strong relationships within the community. Leveraging your skills in diagnostics, treatment planning, and health promotion, you will work closely with our interdisciplinary team to achieve superior patient outcomes and elevate the standard of care in geriatric medicine.

Duties:

1. Clinical Patient-Facing Duties:

  • Conduct comprehensive medical assessments, including physical examinations and medical history reviews, for residents in the assisted living facility.
  • Make accurate diagnoses based on assessments and collaborate with the healthcare team to develop appropriate care plans. Implem
  • ent and coordinate healthcare interventions, including medication management, wound care, chronic disease management, and health promotion activities.
  • Provide education to residents, families, and staff on disease prevention, self-care techniques, and the management of chronic conditions.
  • and manage chronic illnesses, adjusting treatment plans and medications as necessary, in collaboration with physicians and specialists.
  • Engage with patients, families, and staff to create an environment of trust, ensuring open lines of communication to discuss care plans, updates, and changes.

2. Non-Clinical Duties:

  • Required to document, order supplies, participate in meetings, care coordination, and other necessary administrative responsibilities.
  • Ensure accurate and timely completion of administrative tasks, adhering to facility policies and regulatory standards.
  • Participate in monthly evaluations, providing feedback on program outcomes, challenges, and suggestions for improvement.
  • Collaborate with the Clinical Leadership team to identify strategies for enhancing patient care, improving workflow efficiency, and optimizing outcomes.
  • Participate actively in quality improvement projects to continually enhance care and service delivery standards.
  • Act as an advocate for patient-centric care within the clinical leadership team, voicing ideas, concerns, and innovative solutions.
  • Other duties as assigned.

Education and Experience:

  • Board-certified Nurse Practitioner or Physician Assistant – preferably with geriatrics experience
  • Licensed or eligible for licensing in CA.
  • Certification as a Nurse Practitioner/Physician Assistant by a recognized certifying body (e.g., AANP or ANCC).
  • Master’s degree in Nursing, with a specialization in Family or Adult-Gerontology for NPs.
  • Valid DEA Certificate in good standing for Schedule 2-5 prescriptions.
  • Not under current exclusion or sanction by any state or federal health care program, including Medicare or Medicaid, and is not identified as an “excluded person” by the Office of Inspector General of the Department of Health and Human Services or the General Service Administration (GSA), or reprimanded or sanctioned by Medicare. Must maintain Medicare active status.

Pay: $70.00 – $80.00 per hour
