We provide comprehensive primary care with a special emphasis on geriatrics. In certain clinic settings, including Bell, care may also include support for patients with cardiovascular conditions as part of an integrated, physician-led model.

Our model recognizes the unique needs of older adults, including preventive care, chronic disease management, and aging-related issues that meaningfully affect quality of life, such as mobility limitations, cognitive impairment, incontinence, and polypharmacy.

What makes our model distinct is that care is primarily clinic-based, with the ability to continue supporting established patients in the home when clinically appropriate. This allows for continuity and more thoughtful, relationship-based care across settings.


The Job

As an Advanced Practice Provider – Nurse Practitioner (NP) or Physician Assistant (PA) – at Welcome Health, you will provide primary care for older adults as part of a physician-led, multidisciplinary team. This role primarily supports the Bell clinic and focuses on high-quality longitudinal care for patients with a range of medical and social needs.

This is a primary care role with the opportunity to incorporate cardiovascular disease management. The ideal candidate is comfortable managing a primary care panel while also supporting patients with cardiovascular conditions in collaboration with cardiology partners. Cardiologists are available to provide ongoing support, training, and clinical collaboration.

Work Model

  • Full-time role, primarily weekday schedule

  • Primarily clinic-based role supporting the Bell clinic

  • May provide coverage at other Welcome Health clinic locations as needed

  • May perform limited home visits for established patients based on clinical need

  • Mix of primary care visits and cardiovascular-focused follow-up visits based on clinic needs and experience

  • Participates in call coverage as part of the clinical team

  • On-site physician support at the Bell clinic, with additional clinical support available virtually from physician leadership as needed

  • Uses Athena as the electronic medical record platform

  • Competitive base salary with productivity-based bonus tied to wRVU thresholds and performance expectations

What You’ll Do

  • Provide comprehensive primary care for older adults in a physician-led, team-based model

  • Manage a patient panel with a focus on continuity, access, and quality outcomes

  • Diagnose, treat, and manage common acute and chronic conditions, including hypertension, diabetes, heart failure, chronic kidney disease, COPD and other respiratory conditions, and cardiovascular disease within scope

  • Partner closely with physicians, medical assistants, and care team members to implement and adjust care plans

  • Identify and address preventive care gaps, including screenings, vaccinations, fall risk, depression, and cognitive assessment

  • Order and interpret diagnostic and screening tests, and communicate results clearly and compassionately

  • Coordinate care across specialists and support care transitions, including post-hospital follow-up

  • Triage urgent clinical issues and provide appropriate evaluation, treatment, or escalation

  • Perform in-office clinical care and procedures within scope of practice

  • Document clearly and accurately in Athena and use clinical systems effectively

  • Participate in team-based care activities, including huddles, case reviews, and quality improvement work

Your Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program

  • Current NP or PA license in California

  • At least 2 years of experience in primary care, geriatrics, internal medicine, or family medicine

  • Strong foundation in primary care required

  • Experience in cardiology or interest in developing expertise in cardiovascular disease management preferred

  • Ability to travel within the assigned service area, if needed

  • Experience with Athena or similar electronic health record systems preferred

  • Comfort using Microsoft Teams and Microsoft Outlook

  • Spanish fluency strongly preferred. A test will be administered if applicable to assess proficiency
