OverviewSupportive Care Specialist Group is seeking a Palliative Care Nurse Practitioner NP to join our expanding team in Baldwin Park CA providing care across acute care hospitals and LTAC settings.
This role is ideal for an experienced NP who values clinical autonomy meaningful patient relationships and a collaborative interdisciplinary environment. You will primarily care for geriatric patients with complex chronic and serious illnesses while guiding patients and families through important care decisions.
Compensation and Benefits
- $155,000 plus base salary with productivity incentives
- Monday to Friday daytime schedule no overnight shifts
- Limited weekend coverage and light phone based call

Key Responsibilities
- Perform daily patient rounding in hospital and LTAC settings
- Provide palliative care consultations and symptom management
- Conduct goals of care discussions with patients and families
- Develop and implement patient centered care plans
- Collaborate with physicians nurses and interdisciplinary teams
- Maintain timely and accurate EMR documentation
Qualifications
Required
- Active California Nurse Practitioner NP license
- Minimum 2 years inpatient hospital experience
- Minimum 2 years primary care experience
- Strong clinical judgment and ability to work independently
Preferred
- Experience in palliative care hospice or geriatrics
- Bilingual Vietnamese speaking is a plus
Training and Support
- Structured onboarding with shadowing and mentorship
- Ongoing supervision from board certified palliative care physicians
- Support team including social workers chaplains and administrative staff
