Nurse Practitioner / Physician Assistant
Transitions of Care Bridge Clinic

A Transitions of Care Appointment is a follow-up encounter following a visit to the Emergency Department or an inpatient hospital stay. This visit acts as a bridge between the hospital visit and the patient’s long-term care plan. It is an opportunity to extend the care from that provided in these acute settings with a lasting impact on the lives of the patients seen. This Transitions of Care (TOC) resource visit fills a dire need within our current healthcare system.
Clinicians will provide additional support and education while creating a personalized care plan for patients that will involve these elements of our C.A.R.E. goals for each of our patients.
Collection of hospital records to confirm compliance and understanding
Assessment of visit reason diagnosis along with diagnostic screenings and action toward open gaps
Reconciliation of their medications post discharge
Engagement of continued healing through referrals and follow up care
Qualifications
Emergency Medicine-Trained NP or PA, Board Certified with active Arizona license and AZ DEA.
Experience working in Emergency Medicine
