GreenWell Health is looking for a Physician Assistant to join our team in Phoenix, AZ.

As a Physicians Assistant, you will provide primary care to elderly home-bound patients that reside in Assisted Living, Adult Group Home, Memory Care, and Senior Living communities. You will be responsible for delivering exceptional, comprehensive, and personalized care to your patients, enhancing their quality of life and optimizing wellness for as long as possible. The ideal candidate will have a strong understanding of primary care practices with the ability to manage complex health conditions. Successful applicants will possess proven communication skills, the ability to multi-task, manage time effectively, and be able to travel to the patient’s residences. Candidates will also need to collaborate with other physicians, providers, care organizations, and the care team at GreenWell Health.

Duties and Responsibilities:
- Conduct patient visits in Senior Living Communities.
- Assess, diagnose, treat, and manage multiple conditions.
- Perform physical examinations.
- Order labs, diagnostic imaging, and other services.
- Collaborate with your Medical Assistant to coordinate services
- Refill medications.
- Review patient histories.
- Review and manage daily tasks.
- Educate patients, family members, and care staff.
- Collaborate with members of the multidisciplinary team to ensure continuity of care.
- Foster relationships with patients, families, POA/Guardians, and care staff.
- Adhere to all organizational policies, procedures, and regulations.
- Other duties as needed.
Preferred Qualifications:
- Prior experience caring for elderly adults and adults with complex care needs.
- Excellent knowledge of primary care practices.
- Excellent knowledge of diagnostic techniques, wound care, pain management, and Gero-Psych.
- Excellent communication skills.
- Strong organizational skills along with the ability to multi-task and manage time effectively.
- Strong computer and software skills.
Required Qualifications:
– Physician Assistant who is a graduate of an accredited program and has the following:
- National board certification as a Physician Assistant
- Current, unrestricted licensure in Arizona state or ability to obtain licensure within 60 days of hire.
- Active DEA licensure and state-specific controlled substance registration.
- Certification in basic life support (BLS) is required.
- Active, State-issued Driver’s License.
- Reliable transportation with proof of auto insurance.
- Successful completion of a background check.
Job Type: Full-time
Salary: $140,000.00 – $180,000.00 per year
