Umbrella Healthcare, a leading provider of comprehensive primary care, mental health, and palliative care services in Phoenix, Arizona, is seeking a compassionate and dedicated Palliative Care Physician Assistant to join our team. This is a unique opportunity for a professional who has a passion for geriatric care and a commitment to providing end-of-life support.

Key Responsibilities:

Provide palliative care services to patients in various settings, including office days and field visits to SNFs, assisted living facilities, memory care units, and other care facilities.

Conduct comprehensive assessments to determine patients’ palliative care needs.

Develop and implement individualized care plans in collaboration with the healthcare team.

Manage symptoms and provide relief from pain and other distressing symptoms.

Offer guidance and support to patients and their families regarding end-of-life care and decision-making.

Coordinate care with other healthcare professionals to ensure continuity of care.

Document all patient encounters accurately and timely in the electronic medical record system.

Requirements:

Certified Physician Assistant with a valid Arizona license.

Prior experience in hospice and palliative care preferred.

Strong commitment to providing compassionate care to geriatric patients and those facing life-limiting illnesses.

Excellent communication and interpersonal skills, with the ability to work collaboratively in a multidisciplinary team.

Reliable transportation and appropriate insurance coverage as required by Arizona law.

Comfortable with mobile visits and representing Umbrella Healthcare professionally in the field.
