ABOUT THE ROLE
The Nurse Practitioner/Physician Assistant for Primary and Palliative Care is responsible for providing relief from the symptoms of patient’s chronic progressive illnesses and preventing hospitalization through triage and coordination of care. The Nurse Practitioner/Physician Assistant is a mobile provider that is responsible for providing relief from the symptoms of patient’s chronic progressive illnesses and preventing hospitalization through triage and coordination of care.
Core Job Functions:
- Provide advanced primary and palliative care to patients at their residence
- Develop treatment plans to provide symptom management.
- Provide on-call support and work closely with triage team to prevent hospitalizations and provide patient support.
- Collaborate with health care partners and referral sources to promote and sustain community relationships.
- Coordinate with patient’s care team and referral sources to improve patient care and prevent readmissions.
- Arrange for comfort measures such as pain management, home health and spiritual care.
- Assist in end-of-life discussions and hospice transitions, if necessary.
*Monday-Friday, 8:00am-5:00pm*
ROLE REQUIREMENTS
- Master’s degree in nursing from an accredited program with current license to practice professional nursing or graduate from an ARC-PA accredited physician assistant program.
- Board certified advanced nurse practitioner or Physician Assistant
- Active, state license in good standing
- Current DEA
- CPR certification
- Minimum of 2 years of health care experience in hospital, medical outpatient clinic, home health, hospice or specialty care setting.
- Must be a licensed driver with an automobile that is in good working order and demonstrate ongoing insurance in accordance with state and or organization requirements.
