Opportunity Overview

We are seeking a clinically strong, fast-acting Nurse Practitioner or Physician Assistant to join a specialized outpatient clinic in Peoria, Arizona. This role focuses on managing high-acuity patients recently discharged from the hospital, including those recovering from surgery or complex medical events. Who We’re Looking For

You are a confident, detail-oriented advanced practice provider with a strong foundation in acute or complex care. You thrive in settings where critical thinking and rapid decision-making are essential. Whether you’re an experienced provider or a new graduate with advanced acute care training, you are comfortable managing multi-system conditions, navigating transitional care, and communicating clearly with patients and clinical teams alike.

Position Summary

In this role, you’ll provide full-time outpatient care for medically complex patients who have recently transitioned out of inpatient settings. Your goal will be to support recovery, monitor for early signs of complications, and ensure safe continuity of care across clinical environments.

Core Responsibilities

  • Evaluate high-acuity patients recovering from hospitalization or surgery
  • Identify early warning signs and proactively address potential complications
  • Order and interpret labs, imaging, and diagnostic tests
  • Adjust medications and treatment plans as clinically appropriate
  • Develop patient-centered care strategies based on up-to-date evidence and best practices
  • Collaborate with surgeons, physicians, and interdisciplinary team members
  • Counsel patients and caregivers on post-discharge care, red flags, and recovery planning
  • Document all patient interactions and care plans in the electronic health record (EHR)
  • Coordinate follow-up care and referrals to specialists as needed

Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program in the U.S.

(Puerto Rico-based programs not eligible)

  • Current or eligible license to practice in the state of Arizona
  • National board certification (NP or PA-C)
  • Preferred: 3–4 years of clinical experience in acute, hospitalist, urgent care, or similar settings
  • Will consider new graduates with strong acute care rotations and clinical readiness
