This role is ideal for an advanced practice provider seeking to make a measurable impact by preventing hospital readmissions and supporting recovery for complex post-surgical and medically vulnerable populations. You’ll work within a multidisciplinary team focused on early intervention, clinical precision, and continuity of care.

About YouYou are a decisive and clinically adept Nurse Practitioner or Physician Assistant with a strong foundation in acute or post-acute care. You thrive in high-acuity environments and are skilled in identifying complications early, navigating complex treatment plans, and working independently while maintaining close collaboration with referring specialists and surgeons. Whether you’re a seasoned provider or a new graduate with focused acute care training, you are confident managing multi-system issues and committed to delivering patient-centered care.

Position HighlightsThis is a full-time, independent contractor role centered on managing the outpatient needs of medically complex individuals transitioning from hospital to home. Your role will be critical in bridging care gaps, optimizing recovery, and reducing the likelihood of avoidable complications.

Key Responsibilities

  • Conduct clinical assessments for post-surgical and high-acuity patients
  • Monitor for early warning signs of deterioration or complications
  • Order and interpret labs, diagnostics, and imaging as needed
  • Adjust medications and care plans based on clinical findings
  • Develop individualized recovery and follow-up strategies
  • Collaborate with surgeons, specialists, and other care team members
  • Provide education to patients and caregivers regarding recovery and home care
  • Ensure timely and accurate documentation in the EHR system
  • Coordinate referrals and follow-up across the care continuum

Qualifications

  • Graduate of an accredited U.S.-based Nurse Practitioner or Physician Assistant program (Puerto Rico-based programs not eligible)
  • Active or eligible Arizona license as an NP or PA
  • National board certification (NP or PA-C)
  • Preferred: 3–4 years of experience in acute care, hospitalist, or urgent care settings
  • Strong consideration will also be given to new graduates with intensive acute care rotations and demonstrated clinical readiness
