You’ll work with a collaborative, fast-moving team where clinical precision and proactive treatment can significantly reduce readmissions and improve outcomes.

Who You AreYou are a clinically confident and quick-thinking Nurse Practitioner or Physician Assistant with a strong background in acute or complex care. Whether you’re a seasoned provider or a newly trained graduate with advanced acute care rotations, you are decisive under pressure, capable of managing multi-system conditions, and comfortable practicing with autonomy in a post-acute setting. Your communication and documentation skills are sharp, and your commitment to patient-centered care drives your clinical approach.

Position SummaryThis full-time opportunity focuses on managing complex, recently discharged patients in an outpatient care setting. Your primary objective will be to support recovery, reduce the risk of complications, and facilitate smooth care transitions across the healthcare continuum.

Key Duties & Responsibilities

  • Evaluate patients recovering from surgery or hospitalization and manage high-acuity concerns
  • Monitor for early signs of deterioration or complications
  • Order, review, and interpret diagnostic tests and imaging
  • Adjust medications and treatment plans as clinically appropriate
  • Create individualized care strategies based on patient needs and current evidence
  • Work closely with surgeons, physicians, and interdisciplinary team members
  • Counsel patients and families on recovery expectations, warning signs, and home management
  • Complete timely and accurate EHR documentation
  • Ensure proper coordination with specialists and follow-up providers

Qualifications

  • Graduate of an accredited U.S.-based Nurse Practitioner or Physician Assistant program (Puerto Rico programs excluded)
  • Current or eligible Arizona licensure as NP or PA
  • National certification (NP or PA-C)
  • Preferred 3–4 years of experience in acute, inpatient, or urgent care settings
  • New grads with focused acute care rotations and strong clinical readiness will also be considered
