You excel at managing transitions, identifying complications early, and communicating effectively with patients, caregivers, and care teams.
Position Overview
In this role, you’ll manage high-risk patients recently discharged from the hospital or recovering from surgery. Your clinical responsibilities will focus on the early identification of complications, care coordination, and ensuring stability as patients navigate the critical period following hospitalization.
Key Responsibilities
- Perform detailed assessments and manage acute and subacute medical issues
- Monitor surgical wounds, medication side effects, and potential readmission risks
- Order and interpret labs, imaging, and other diagnostics
- Prescribe medications and monitor response in accordance with licensure
- Develop, adjust, and execute evidence-based care plans
- Collaborate with surgeons, hospitalists, specialists, and case managers
- Educate patients and families on recovery, complications, and self-management
- Document all patient encounters accurately and promptly in the EHR
- Coordinate timely follow-up and handoffs between care settings
Qualifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant program (U.S.-based; excludes Puerto Rico)
- Active Arizona license or eligibility to obtain
- National board certification (NP or PA-C)
- Ideally 3–4 years of relevant experience in acute, urgent, or inpatient care
- Strong clinical new graduates with advanced acute care training also considered
