Position Overview
A well-regarded outpatient care center in Peoria, AZ is actively seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to lead transitional care services for patients recently discharged from hospitals and acute care facilities. This full-time position, structured as a 1099 independent contractor, is ideal for seasoned clinicians who thrive in high-acuity settings and are ready to manage complex patient needs autonomously.
You will play a central role in guiding patients through vulnerable post-discharge periods, focusing on risk reduction, care continuity, and improved recovery outcomes.

Key Responsibilities
- Perform in-depth assessments and follow-ups for recently discharged patients
- Manage complex chronic conditions and acute medical issues in the outpatient setting
- Order, interpret, and act on labs, imaging, and diagnostics
- Adjust medications and treatment plans as needed
- Provide comprehensive education to patients and caregivers regarding post-hospital care
- Coordinate closely with primary care, specialty providers, and community-based services
- Initiate referrals for home health, rehab, palliative care, or social services
- Document all clinical interactions in the EMR accurately and efficiently
- Participate in quality improvement initiatives focused on reducing hospital readmissions
Candidate Profile
You are a highly capable provider with strong critical thinking skills and a background in acute or complex care environments. You excel in independent practice, value patient-centered care, and are driven by measurable impact in transitional medicine.
Required Qualifications
- Graduate of an accredited NP or PA program (U.S.-based only)
(Note: Programs in Puerto Rico are not eligible)
- Active or eligible Arizona state license (NP or PA)
- National board certification (FNP, AGACNP, or PA-C)
- Skilled in EMR systems with a commitment to timely documentation
- Excellent communication and coordination skills across clinical teams
Preferred Background
- 3–4 years of experience in acute care, hospital medicine, urgent care, or transitional care
- Recent graduates with relevant clinical rotations or postgraduate acute care training may be considered
