Position Summary

A high-performing outpatient clinic in Peoria, AZ is seeking a full-time Nurse Practitioner or Physician Assistant to manage high-acuity patients recently discharged from hospital care. As an independent contractor, you will be central in bridging the gap between inpatient and home recovery—delivering early intervention, minimizing readmissions, and supporting complex care plans.

This is a clinically demanding and highly autonomous role, well suited for providers who are confident operating at the top of their license and committed to improving transitional outcomes.

Key Responsibilities

  • Conduct in-depth patient evaluations following hospital discharge
  • Manage acute and chronic conditions during transitional recovery
  • Interpret and act on diagnostic studies and lab work
  • Adjust medications and interventions in real time based on patient status
  • Collaborate with referring specialists, PCPs, and care teams to ensure continuity
  • Educate patients and caregivers on care protocols, medication use, and symptom monitoring
  • Coordinate post-discharge follow-ups and ancillary service referrals
  • Accurately document all patient interactions in the EMR system
  • Participate in clinical quality initiatives aimed at reducing readmission rates

Candidate Profile

You are clinically sharp, detail-oriented, and excel in high-acuity, outpatient care settings. You approach complex medical presentations with confidence, communicate clearly with patients and interdisciplinary teams, and bring a proactive mindset to transitional medicine.

Minimum Qualifications

  • Graduation from a U.S.-based, accredited Nurse Practitioner or Physician Assistant program

(Note: Puerto Rico-based programs are not eligible)

  • Active or eligible Arizona state license as an NP or PA
  • National certification (FNP, AGACNP, or PA-C)
  • Skilled in EMR systems and timely, thorough clinical documentation
  • Excellent clinical reasoning, communication, and care coordination abilities

Preferred Background

  • 3–4 years of experience in acute care, hospital medicine, urgent care, or transitional care
  • Will consider recent graduates with relevant clinical rotations or postgraduate training in acute care or hospitalist medicine
