Position Overview

We are seeking a skilled and self-directed Nurse Practitioner or Physician Assistant to join our outpatient clinic in Peoria, Arizona, focused on managing high-acuity patients following hospital discharge. This full-time 1099 independent contractor position offers autonomy, a high-impact patient panel, and compensation based on productivity and clinical performance.

You’ll play a central role in reducing hospital readmissions by providing timely, in-depth care to patients navigating complex post-acute recovery. The position is ideal for advanced practice providers comfortable practicing at the top of their license in fast-paced, clinically intensive environments.

What You’ll Be Doing

As part of a dedicated team, you will oversee care for patients transitioning from hospital to home—individuals often presenting with multiple chronic and acute conditions. Your expertise in early intervention, medication management, and patient education will be critical to achieving optimal outcomes.

Key Responsibilities

  • Perform comprehensive clinical assessments for post-discharge patients
  • Monitor recovery progress and identify early signs of clinical deterioration
  • Order, interpret, and respond to labs, diagnostics, and imaging studies
  • Adjust care plans and medication regimens in real time based on patient needs
  • Coordinate with referring physicians, specialists, and interdisciplinary care teams
  • Educate patients and caregivers on symptom monitoring, medications, and home care
  • Manage referrals, follow-up scheduling, and care transitions
  • Document all clinical interactions accurately and promptly in the EHR
  • Participate in continuous improvement efforts focused on patient outcomes

Who You Are

You are a confident, highly organized provider with strong critical thinking and clinical judgment. You thrive in transitional care, understand the nuances of managing high-risk patients, and excel in proactive care models that emphasize prevention over reactivity.

Minimum Requirements

  • Graduate of an accredited U.S.-based Nurse Practitioner or Physician Assistant program

(Note: Programs based in Puerto Rico are not eligible)

  • Current or eligible Arizona licensure as a Nurse Practitioner or PA
  • National board certification (e.g., PA-C, FNP-C, AGACNP, etc.)
  • Proficiency in electronic health record systems and timely documentation
  • Excellent diagnostic reasoning, communication, and patient education skills

Preferred Qualifications

  • 3–4 years of experience in acute care, hospitalist medicine, urgent care, or transitional/post-acute care
  • Recent graduates with advanced training in acute care will also be considered
