Position OverviewWe are seeking an experienced and motivated Nurse Practitioner or Physician Assistant to join a fast-paced outpatient clinic in Peoria, Arizona, specializing in the management of high-acuity patients following hospital discharge. This full-time independent contractor position is ideal for a provider who is confident practicing at the top of their license and passionate about delivering proactive care that prevents hospital readmissions.
You’ll work in a dynamic environment focused on early intervention, care coordination, and patient education—all essential components to improving outcomes for medically complex patients.
What You’ll Be DoingAs a vital member of our clinical team, you will manage post-discharge patients presenting with multiple acute and chronic conditions. These patients require high-touch, comprehensive assessments and close monitoring to support their recovery journey and reduce readmission risk.
Key Responsibilities
- Conduct thorough clinical evaluations for recently discharged patients
- Monitor patient recovery and identify signs of clinical decline
- Order, interpret, and act on lab results, diagnostics, and imaging
- Adjust medications and care plans in response to clinical findings
- Collaborate with referring physicians, specialists, and care teams
- Educate patients and families on red flag symptoms, medication adherence, and self-care practices
- Coordinate follow-up care and specialist referrals
- Maintain detailed, timely documentation in the electronic health record (EHR)
- Contribute to a proactive care model focused on long-term patient outcomes
Who You AreYou are a confident, analytical, and compassionate provider with a strong foundation in acute or transitional care. You understand the complexities of medically fragile patients and excel at clinical decision-making in outpatient settings that require close monitoring and timely intervention.
Minimum Requirements
- Graduate of an accredited U.S.-based Nurse Practitioner or Physician Assistant program
(Note: Programs based in Puerto Rico are not eligible)
- Licensed or license-eligible in the state of Arizona
- National certification as a Nurse Practitioner or Physician Assistant (PA-C)
- Proficiency with EHR systems and thorough clinical documentation
- Strong clinical reasoning, communication, and problem-solving skills
Preferred Qualifications
- 3–4 years of experience in acute care, hospitalist, urgent care, or post-acute environments
- New graduates with advanced acute care training will also be considered
