Position SummaryA high-performing outpatient clinic in Peoria, AZ is seeking a Nurse Practitioner or Physician Assistant to lead transitional care management for patients recently discharged from acute hospital settings. This is a full-time, independent contractor role designed for experienced providers who are comfortable practicing autonomously and navigating complex post-discharge care plans.

You will play a critical role in reducing readmissions, closing care gaps, and ensuring safe and effective transitions back to the home environment.

Core Responsibilities

  • Perform detailed clinical evaluations for recently discharged patients
  • Monitor and manage complex chronic and acute medical conditions
  • Interpret labs, diagnostics, and imaging; adjust care plans accordingly
  • Modify medications and treatments based on evolving patient status
  • Educate patients and caregivers on post-hospital care instructions
  • Coordinate closely with primary care providers and specialists
  • Initiate referrals to home health, rehab, or palliative services as needed
  • Accurately document all care interactions within the EMR system
  • Contribute to quality improvement efforts focused on reducing readmissions

Ideal Candidate ProfileYou are a confident, detail-oriented provider with excellent clinical judgment and a strong foundation in acute care. You’re comfortable with independent practice, excel at interdisciplinary collaboration, and value the opportunity to support high-risk patients during a critical recovery window.

Minimum Requirements

  • Graduate of a U.S.-based, accredited NP or PA program

(Programs located in Puerto Rico are not eligible)

  • Active or eligible Arizona NP or PA license
  • National board certification: FNP, AGACNP, or PA-C
  • Skilled in EMR usage and thorough, timely documentation
  • Strong clinical reasoning, communication, and care coordination skills

Preferred Experience

  • 3–4 years in hospital medicine, urgent care, acute care, or transitional care settings
  • Recent graduates with relevant clinical rotations or postgraduate training in acute or transitional care will be considered
