Role SummaryAs a weekend provider, you will deliver hands-on medical care in skilled nursing and post-acute care environments. You will be responsible for monitoring changes in condition, managing medications, and coordinating follow-up care for patients with complex health needs. Your contributions will play a critical role in minimizing unnecessary hospitalizations and ensuring high standards of care.
Who We’re Looking ForWe are seeking a confident and self-directed Nurse Practitioner or Physician Assistant with prior experience in post-acute or long-term care. You excel in independently managing clinical responsibilities, communicating effectively across interdisciplinary teams, and maintaining detailed documentation. A passion for geriatric care and strong decision-making abilities are key to success in this role.
Key Responsibilities
- Provide on-site weekend medical care at skilled nursing facilities
- Assess, diagnose, and manage both acute and chronic conditions
- Adjust treatment plans and medications as needed based on patient status
- Collaborate with nursing staff, therapy teams, and case managers to ensure cohesive care
- Communicate care updates to patients, families, and facility staff
- Maintain accurate, timely clinical documentation in the EMR system
- Reduce avoidable hospital admissions through proactive care coordination
Qualifications
- Active, unrestricted license as a Nurse Practitioner or Physician Assistant in Arizona
- Graduate of an accredited U.S.-based NP or PA program (Puerto Rico-based programs are not eligible)
- Previous experience in skilled nursing, post-acute, or long-term care settings preferred
- Proficiency with electronic medical record systems
- Strong clinical judgment, organizational skills, and interpersonal communication
