Opportunity SummaryWe are seeking a dependable and experienced Advanced Practice Provider to deliver high-quality care in skilled nursing facilities throughout Mesa, Gilbert, and Chandler. This part-time weekend role offers a rewarding way to contribute to patient care without interfering with your weekday commitments. You’ll help bridge continuity of care during weekends, working independently while also benefiting from a strong support team.
Role OverviewThis position involves direct clinical oversight of residents in skilled nursing facilities. You’ll evaluate changes in condition, manage medications, and provide follow-up care while ensuring residents receive appropriate, timely medical attention. Your contributions will directly support reduced hospitalizations and improved patient experiences.
Ideal CandidateYou are a clinically confident NP or PA with experience in post-acute or long-term care. You thrive in autonomous roles, communicate clearly across multidisciplinary teams, and are committed to providing compassionate, timely care for older adult populations. Strong documentation skills and the ability to make independent clinical decisions are essential in this role.
Primary Responsibilities
- Deliver in-person weekend medical care in designated SNF facilities
- Evaluate and treat acute and chronic conditions in medically complex patients
- Adjust medications and care plans in response to evolving clinical needs
- Coordinate closely with nurses, therapists, and case managers to align care delivery
- Provide clear communication to patients, families, and facility staff
- Maintain accurate, regulatory-compliant documentation in the EMR
- Support care coordination and reduce avoidable hospital transfers
Qualifications
- Current, unrestricted license as a Nurse Practitioner or Physician Assistant in Arizona
- Graduate of a U.S.-accredited NP or PA program (Puerto Rico-based programs not eligible)
- Experience in post-acute or long-term care settings preferred
- Proficient in using electronic medical records systems
- Excellent clinical judgment, organizational skills, and communication abilities
