Position Overview

This is an ideal opportunity for an experienced Nurse Practitioner or Physician Assistant seeking meaningful weekend work in a post-acute care setting. You’ll provide continuity of care for skilled nursing facility residents in Mesa, Gilbert, and Chandler—ensuring quality outcomes while maintaining flexibility in your weekly schedule.

What You’ll Do

As a key weekend provider, you will oversee medical care for residents in SNFs, helping manage complex conditions, respond to acute changes, and collaborate closely with on-site staff to ensure smooth transitions and prevent unnecessary hospital readmissions. Your clinical autonomy will be balanced with access to a knowledgeable and supportive interdisciplinary team.

You are a self-directed and compassionate advanced practice provider who thrives in skilled nursing or post-acute environments. With solid clinical skills and the ability to make confident decisions independently, you bring structure and reliability to weekend care. Your communication is clear, your documentation is prompt and accurate, and you are skilled at managing both acute issues and chronic disease progression in medically vulnerable populations.

Key Responsibilities

  • Provide weekend coverage at SNFs, conducting medical rounds and follow-up visits
  • Assess and manage new or worsening symptoms, chronic conditions, and medication needs
  • Order diagnostics and medications in accordance with patient conditions and goals of care
  • Revise care plans as clinically appropriate
  • Coordinate closely with nursing, therapy, and social services teams
  • Communicate care plans with patients, families, and facility staff
  • Ensure documentation is complete, timely, and compliant with SNF protocols and regulatory standards
  • Maintain a focus on minimizing unnecessary hospital transfers and improving patient comfort

Required Qualifications

  • Active and unrestricted Nurse Practitioner or Physician Assistant license in Arizona
  • Graduate of a U.S.-based accredited NP or PA program (Puerto Rico programs not eligible)
  • Experience in post-acute, SNF, or long-term care environments is strongly preferred
  • Proficient in using electronic medical records (EMR)
  • Strong clinical acumen, professionalism, and independent decision-making ability
