In this role, you will play an essential part in managing medically complex residents during their transitions of care. About You

You are a reliable and clinically skilled Nurse Practitioner or Physician Assistant with a strong interest in geriatric medicine and post-acute care. You are confident managing high-acuity patients and value thorough, timely documentation. You work well independently and as part of an interdisciplinary team, with a clear focus on patient outcomes, communication, and clinical excellence.

Responsibilities

  • Perform initial and ongoing evaluations for patients residing in skilled nursing facilities
  • Respond to acute medical concerns and manage chronic health conditions
  • Order and interpret diagnostic tests, including labs and imaging
  • Adjust medications and update care plans as needed
  • Collaborate with nursing staff, physicians, therapists, and social workers
  • Communicate care goals and discharge planning with patients and families
  • Maintain detailed and prompt documentation in the electronic health record (EHR)
  • Ensure care delivery aligns with regulatory and facility standards

Qualifications

  • Current and unrestricted Arizona license as a Nurse Practitioner or Physician Assistant
  • Graduate of a U.S.-based, accredited NP or PA program (Puerto Rico programs excluded)
  • At least 1–2 years of experience in SNF, long-term care, or post-acute care is preferred
  • Proficiency with EMR systems and clinical documentation
  • Strong problem-solving, organizational, and communication abilities
