What We’re Looking For:You are a dependable and experienced NP or PA who thrives in the skilled nursing and post-acute environment. You bring excellent clinical judgment, are confident in managing a variety of complex medical conditions, and can work both independently and in partnership with facility care teams. You value accuracy in documentation, thoughtful communication, and a hands-on approach to patient care.

Role Summary:As the weekend provider, you’ll serve as a key member of our post-acute care team, supporting patients in skilled nursing facilities through timely assessments, care management, and proactive coordination. You’ll help reduce unnecessary hospitalizations and enhance quality of life for patients transitioning through levels of care.

Key Responsibilities:

  • Conduct initial and follow-up evaluations for SNF residents
  • Manage acute changes in condition and provide chronic disease oversight
  • Order and interpret labs, imaging, and diagnostic testing
  • Prescribe medications and update treatment plans based on patient status
  • Collaborate with attending physicians, nurses, rehab therapists, and case managers
  • Facilitate communication with patients, families, and staff regarding care and discharge needs
  • Ensure timely, accurate documentation in the electronic medical record (EMR)
  • Adhere to facility protocols and all regulatory requirements

Qualifications:

  • Active, unrestricted Nurse Practitioner or Physician Assistant license in Arizona
  • Graduate of a U.S.-accredited NP or PA program (excluding Puerto Rico)
  • 1–2 years of clinical experience in SNF, long-term care, or post-acute settings preferred
  • Strong EMR documentation skills and ability to work independently
  • Excellent decision-making, communication, and organizational skills
