About the RoleWe are currently seeking a skilled and compassionate Nurse Practitioner or Physician Assistant to provide weekend care at skilled nursing and post-acute care facilities in the Mesa, Gilbert, and Chandler areas. This independent contractor position offers the flexibility of a part-time weekend schedule while allowing you to make a meaningful clinical impact in a post-acute setting.
Position SummaryAs a weekend advanced practice provider, you will deliver comprehensive care for patients in skilled nursing environments—managing chronic and acute conditions, supporting transitions of care, and contributing to a multidisciplinary care team. This role offers clinical autonomy with the support of nursing staff and physicians, focusing on continuity, quality outcomes, and hospital avoidance.
Key Responsibilities
- Perform in-depth patient assessments and formulate individualized treatment plans
- Manage chronic conditions and acute concerns in collaboration with facility teams
- Order and interpret labs, imaging, and diagnostic testing as needed
- Prescribe medications and adjust treatment protocols based on patient response
- Provide education and guidance to patients, families, and caregivers
- Coordinate with nurses, physicians, case managers, and other care providers
- Conduct in-scope procedures (as applicable) to support patient care
- Accurately document encounters in the EMR in a timely manner
- Ensure care delivery aligns with state and federal regulatory standards
- Facilitate referrals and post-discharge follow-up when appropriate
Requirements
- Active, unrestricted Arizona license as a Nurse Practitioner or Physician Assistant
- Graduate of an accredited U.S.-based NP or PA program
(Note: Programs based in Puerto Rico are not eligible)
- 1–2 years of experience in post-acute, long-term care, or skilled nursing preferred
- Proficient in EMR systems and familiar with SNF workflows
- Strong clinical reasoning, documentation, and communication skills
