Job Title: Nurse Practitioner / Physician Assistant

Location: Mesa / Gilbert / Chandler, AZ

Hours & Schedule: Part-Time (Saturday & Sunday, 8:00 AM – 5:00 PM)

Work Environment: Skilled Nursing Facility (Post-Acute Care)

Travel Required: No

Salary / Hourly Rate: $65–$70 / hour

Bonus Offered: N/A

Job Summary:

We are seeking a Nurse Practitioner or Physician Assistant to deliver comprehensive care in a skilled nursing facility setting across Mesa, Gilbert, and Chandler, AZ. In this part-time, weekend-only role, you’ll assess and treat post-acute patients in collaboration with a multidisciplinary team. This is an excellent opportunity for a provider looking to make a meaningful impact while maintaining a flexible schedule.

Job Duties & Responsibilities:

  • Conduct health assessments and develop patient-centered care plans
  • Diagnose and manage acute and chronic medical conditions
  • Order and interpret labs, imaging, and other diagnostic tests
  • Prescribe medications and adjust treatments as necessary
  • Educate patients and families on care plans and preventive health
  • Collaborate with physicians, therapists, and nursing staff
  • Perform minor procedures within scope of practice
  • Document patient care in the electronic medical record (EMR)
  • Refer to specialists and coordinate follow-up care
  • Ensure compliance with facility and regulatory standards

Prerequisites / License & Certification Requirements:

  • Active NP or PA license in Arizona
  • Accredited U.S.-based NP or PA program (excluding Puerto Rico)
  • 1–2 years post-acute or SNF experience preferred
