The only housecall based concierge (DPC-Direct Primary Care) practice in the East Valley. Providing housecalls to families in the Chandler, Gilbert, Mesa, Queen Creek and San Tan Valley area. Must have 4+ years in family practice or pediatric setting.

Key Responsibilities:
- Patient-Centric Care: Uphold our commitment to patient-centered care by actively involving patients and their families in shared decision-making processes, fostering trust, and building strong relationships.
- Housecall Services: Provide comprehensive medical services to patients of all ages, including pediatrics, family practice, and urgent care, within the comfort and convenience of their homes.
- Patient Accessibility: Our providers are available to our patients during weekday hours via company provided cell phone.
- Diagnostic Proficiency: Demonstrate sound clinical judgment and diagnostic skills to accurately assess and diagnose various medical conditions across different age groups.
- Treatment Planning: Develop individualized treatment plans that align with patients’ preferences, medical history, and current health status, ensuring the best possible outcomes.
- Emergency Response: Manage urgent medical situations effectively, delivering timely and appropriate care in emergency cases.
- Collaborative Approach: Collaborate with a multidisciplinary team, including specialists, support staff, and colleagues to ensure seamless patient care and a holistic approach to health management.
- Health Promotion and Education: Educate patients and their families on disease prevention, healthy lifestyle choices, and proper management of chronic conditions.
- Telemedicine Integration: Incorporate telemedicine technologies where appropriate to offer virtual consultations, follow-ups, and support for patients.
- Documentation: Maintain accurate and detailed medical records, ensuring compliance with all legal and regulatory requirements.
- Continuous Learning: Stay updated with the latest medical advancements, research, and best practices to enhance your clinical expertise and provide the highest level of care.
Qualifications and Requirements:
- Medical degree (PA/NP) from an accredited institution.
- Active and unrestricted medical license in the state.
- Up to 40 hours per week of in person visits and patient communications
- 4+ years in primary care or pediatrics.
- Proven experience in patient-centric care and shared decision-making.
- Strong diagnostic skills and the ability to develop effective treatment plans.
- Excellent communication and interpersonal skills.
- Empathy and compassion towards patients of all ages.
- Ability to work independently and as part of a team.
- Familiarity with telemedicine tools and technology.
- Commitment to maintaining confidentiality and ethical standards.
