Full Job Description
The Family Nurse Practitioner (FNP) provides comprehensive primary care services to adult and geriatric patients, including Veterans receiving care through VA-authorized services, as well as established and walk-in primary care patients. The FNP focuses on preventive care, chronic disease management, and routine medical evaluation in an outpatient primary care and wellness setting.
Key Responsibilities
- Provide comprehensive primary care services, including health assessments, physical exams, and medical histories
- Deliver care to VA patients in accordance with VA-authorized primary care guidelines and documentation standards
- Evaluate and manage common acute, subacute, and chronic conditions appropriate for a primary care setting
- Manage chronic diseases such as hypertension, diabetes, hyperlipidemia, COPD, and other long-term conditions
- Provide preventive care, wellness visits, and health maintenance screenings
- Order, interpret, and review laboratory tests and diagnostic studies
- Prescribe medications and treatment plans within state scope of practice
- Educate patients on disease prevention, lifestyle modification, and treatment adherence
- Maintain accurate, timely, and compliant documentation in the electronic medical record (EMR)
- Coordinate care with VA services, specialists, and community providers as needed
- Adhere to all federal, state, VA, and organizational policies and standards of care
Qualifications
- Master’s or Doctorate degree from an accredited Nurse Practitioner program
- Current Family Nurse Practitioner (FNP) certification
- DEA license (or ability to obtain)
- Current BLS certification
- Experience in primary care, VA/community care, or outpatient clinic settings preferred

Job Type: Part-time
Pay: $60.00 – $65.00 per hour
