Position Title: Nurse Practitioner (NP) or Physician’s Assistant (PA), HIV Prevention

Reports To: Director, Healthcare Operations Southern Region

Department: Clinic

Location: Montgomery, AL.

Position Purposes: The PrEP Provider at Five Horizons Health Services is responsible for delivering high-quality, culturally competent care focused on HIV prevention through PrEP (Pre-Exposure Prophylaxis). This role involves comprehensive sexual health screenings, evaluation, and treatment of sexually transmitted infections (STIs), along with providing PrEP consultations and monitoring. The PrEP Provider will collaborate closely with their collaborating physician and clinical staff to ensure optimal patient outcomes and adherence to healthcare protocols.

Essential Functions:

· Complete sexual health screenings, evaluation, and treatment of sexually transmitted infections, PrEP consultations, and monitoring, provide limited primary care services, HIV medical services, and develop interventions for at-risk individuals using clinical decision-making skills to promote positive health outcomes.

· Meet with patients promptly to complete intake, sexual history, provide testing, PrEP evaluation and monitoring, limited primary care, and offer appropriate treatment and education aligned with nationally and locally recognized recommendations.

· Work collaboratively with the clinical team, consulting with and reporting STD/STI infections as required. Work closely with clinical staff members to coordinate and obtain needed services for patients.

· Document all findings in the EMR system promptly according to FHHS standards.

· Develop appropriate, patient-specific interventions for risk reduction behaviors. Screen and refer clients to other community service providers as appropriate. Review lab results, ensure treatment completion, and communicate results to patients.

· Ensure timely execution of treatment orders.

· Assist in the development of healthcare protocols, procedures, and standing orders as appropriate to standardize and enhance care.

· Educate staff, patients, and the community on sexually transmitted infections, treatment options, and interventions.

· Use and disclose patients’ protected health information in compliance with Health Insurance Portability and Accountability Act (HIPAA) standards.

· Fulfill duties assigned by the Chief Medical Systems Officer, Director Healthcare Operations Southern Region, or the Chief Operating Officer.

· Integrate subspecialist recommendations into patient treatment plans.

· Provide telemedicine services as needed.

· Maintain an active nursing license and complete continuing education as required by Alabama state licensing standards.

· Participate in off-site training and meetings with pharmaceutical vendors as necessary.

· Precept nursing and medical students when required.

· Supervise and meet with clinical staff to ensure compliance with clinic protocols and FHHS policies.

· Make policy recommendations to enhance clinical service effectiveness and efficiency.

· Document patient records accurately and comprehensively, adhering to organizational standards.

· Maintain collaborative agreements and meet all requirements of the Alabama Board of Nursing or Board of Medical Examiners.

· Engage in systemwide educational, clinical effectiveness, and quality improvement activities.

· Assist with medical management paperwork, billing, and credentialing requirements in a timely manner.

· Participate in practice management reviews with the Chief Medical Systems Officer and Director, Healthcare Operations Southern Region.

· Assume additional responsibilities assigned by the Chief Operating Officer, Chief Medical Systems Officer, or Director, Healthcare Operations Southern Region.

Education & Experience:

· State License as an APRN or Physician Assistant

· Certification as an APRN through the AANP, ANCC, or NCCPA

· DEA license, or willingness to obtain upon hire

· Minimum of 2 years of clinical experience as a nurse practitioner.

· Effective communication and interpersonal skills.

· Timely completion of continuing education necessary for state licensure.

· Current CPR certification.

Qualifications & Requirements:

· Knowledge of sexually transmitted diseases, transmission, and treatment.

· Familiarity with PrEP/PEP and HIV.

· Understanding of HIPAA laws and Alabama State Department of Health treatment protocols.

· Preferred experience in PrEP/PEP/HIV care and treatment.

· Ability to provide compassionate care without judgment.

· Capability to collaborate effectively with diverse staff, patients, and families.

· Ability to work independently with minimal supervision

· Ability to prioritize and organize work assignments

· Proficiency in EMR, Excel, MS Word, and PowerPoint.

Job Type: Full-time

Pay: From $105,000.00 per year
