Full Job Description
We are seeking a
**Nurse Practitioner, Advanced Practice Provider**
to deliver effective, equitable, value-based primary care to adult Medicare patients.
In this role, you will see patients independently, conduct office visits for routine and acute issues, perform wellness visits and health assessments, and coordinate care with specialists and other providers. You will build meaningful patient relationships, educate patients on their health, participate in care coordination, and support population health initiatives.
Responsibilities include care delivery within a collaborative outpatient team, conducting home visits when needed, and assisting with phone triage and outreach. You will report to the Center Medical Director and work closely with operational leadership.
Qualifications include a Master of Science in Nursing, Doctor of Nursing Practice, or NP certification, active state NP license, DEA license, and US work authorization. Preferred experience includes primary care, familiarity with HCC documentation, ICD-10 coding, and health risk assessments, along with a passion for teamwork and population health. Bilingual skills are a plus.
