Overview
We are seeking an experienced Nurse Practitioner to join our expanding team in Brooklyn, NY. This is an in-person position where you’ll provide care through a telehealth platform in a modern, collaborative clinical environment. Your expertise will help improve patient outcomes through comprehensive assessments, accurate diagnoses, and effective treatment plans.
Responsibilities
- Conduct patient evaluations and follow-up visits through a telemedicine platform
- Diagnose, treat, and manage a variety of acute and chronic conditions
- Develop and implement individualized treatment plans
- Maintain accurate and timely electronic medical records
- Collaborate with physicians and clinical staff to ensure exceptional patient care
- See approximately 25 patients per day
Qualifications
- Active Nurse Practitioner/ Physian Assistant license in New York
- Must be credentialed with both Medicare and Medicaid
- Strong clinical assessment and communication skills
- Telehealth experience is a plus
- Ability to thrive in a fast-paced, patient-focused environment
Compensation & Benefits
- $150,000–$200,000 annual salary
Pay: $150,000.00 – $200,000.00 per year
