Nurse Practitioner (NP) / Physician Assistant (PA) – Telehealth & Hybrid Primary Care Provider

Overview
JaViish Medical Group is seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing healthcare team. This role provides an exciting opportunity to deliver high-quality, patient-centered care through telehealth and in-person visits while working collaboratively with our medical director and healthcare team.
The ideal candidate will provide comprehensive primary care services with a focus on preventative medicine, weight management, obesity medicine, hormone therapy, men’s health, women’s health, and chronic disease management. The provider will evaluate patients, develop individualized treatment plans, educate patients, and help improve overall health outcomes through evidence-based medical care.
This position operates under the supervision and guidance of JaViish Medical Group’s Medical Director. The selected provider must complete and maintain all required collaborative practice agreement (CPA) documentation, credentialing requirements, and compliance standards prior to providing patient care.
Compensation
- $35 per completed and signed patient encounter
- Compensation is based on successfully closed encounters with complete documentation and provider signature.
- Flexible scheduling opportunities available based on patient volume and clinic needs.
Provider Requirements
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state(s) where services are provided.
- Must maintain an active DEA license (if prescribing controlled medications).
- Must sign and maintain a Collaborative Practice Agreement (CPA) as required by applicable state regulations.
- Must obtain and maintain individual professional malpractice insurance coverage.
- Must provide proof of current licensure, certifications, and required credentials.
- Must comply with all JaViish Medical Group clinical policies, HIPAA requirements, and documentation standards.
Responsibilities
- Conduct comprehensive patient evaluations through telehealth and/or in-person visits.
- Review patient medical history, medications, allergies, symptoms, and health concerns.
- Perform assessments, develop diagnoses, and create individualized treatment plans.
- Manage acute and chronic medical conditions.
- Provide preventative healthcare services and health education.
- Manage patients participating in medical weight management programs, including obesity treatment and lifestyle modification support.
- Evaluate and manage patients receiving hormone therapy services, including men’s and women’s health concerns.
- Provide guidance regarding nutrition, exercise, wellness, and disease prevention.
- Order and review appropriate laboratory testing and diagnostic studies.
- Prescribe medications and treatments according to clinical guidelines, state regulations, and medical director oversight.
- Monitor patient progress and adjust treatment plans as medically appropriate.
- Provide patient education regarding medications, treatment plans, and long-term health goals.
- Complete accurate and timely documentation within the electronic medical record (EHR) system.
- Ensure all encounters are properly documented, coded, and signed before closing the patient visit.

Documentation & Compliance Expectations
The provider must:
- Maintain complete and accurate medical records.
- Follow HIPAA compliance standards.
- Document all patient encounters according to JaViish Medical Group protocols.
- Complete required coding documentation, including appropriate CPT and ICD-10 coding.
- Sign and close encounters within the required timeframe.
- Maintain professional communication with patients and the healthcare team.
Preferred Experience
Preferred candidates will have experience in:
- Primary care or outpatient medicine
- Telehealth medicine
- Obesity medicine or weight management
- Hormone replacement therapy
- Chronic disease management
- Preventative healthcare
- Patient education and health coaching
- Electronic health record systems
- Medical documentation and coding
Experience with weight loss medications, hormone therapy, wellness programs, or functional medicine is highly preferred.
