Position Summary

The Physician Assistant provides comprehensive, patient-centered primary care to individuals and families served by the health center, including pediatric, adolescent, adult, and geriatric patients. The PA evaluates, diagnoses, treats, and manages acute illnesses, chronic conditions, preventive health needs, and minor injuries within the provider’s education, training, Texas scope of practice, delegated authority, and approved clinical privileges. The position requires a strong commitment to medically underserved and culturally diverse populations and to the mission of an FQHC providing accessible, high-quality community care.

Essential Duties and ResponsibilitiesDirect Patient Care

· Conduct comprehensive and problem-focused histories and physical examinations; assess risk factors; develop differential diagnoses; and establish evidence-based treatment plans.

· Evaluate, diagnose, and treat common acute and chronic conditions affecting patients across the lifespan, including hypertension, diabetes, hyperlipidemia, obesity, asthma/COPD, thyroid disorders, respiratory infections, gastrointestinal and genitourinary disorders, musculoskeletal conditions, dermatologic conditions, and common behavioral health concerns.

· Order, perform, review, and interpret appropriate laboratory tests, EKGs, X-rays, ultrasounds, and other diagnostic studies within approved privileges.

· Prescribe medications and therapeutic interventions in accordance with Texas law, clinic policy, delegated authority, and applicable prescriptive authority arrangements.

· Monitor treatment response, medication safety, adherence, and health outcomes; revise care plans when clinically indicated.

· Identify patients requiring emergency evaluation, hospitalization, specialty consultation, or a higher level of care and coordinate timely transfer or referral.

· Maintain an efficient patient flow while preserving quality, safety, dignity, and appropriate clinical documentation.

Preventive and Community Health

· Perform well-child visits, adolescent health visits, annual adult examinations, sports and school physicals, and other preventive assessments.

· Provide age-appropriate immunizations, screenings, and counseling consistent with current clinical guidelines and the health center’s scope of services.

· Educate patients and families regarding diagnoses, medications, nutrition, exercise, disease prevention, treatment adherence, and self-management.

· Screen for health-related social needs and barriers such as transportation, food insecurity, housing instability, medication affordability, language needs, and difficulty accessing specialty care.

· Deliver culturally responsive care that considers the patient’s language, health literacy, values, socioeconomic circumstances, and support system.

· Support the health center’s mission of improving access and health outcomes regardless of a patient’s insurance status or ability to pay.

Care Coordination and Collaboration

· Collaborate with physicians, nurse practitioners, nurses, medical assistants, behavioral health professionals, pharmacists, care coordinators, social-service staff, and community partners.

· Coordinate specialty referrals, diagnostic services, hospital follow-up, pharmacy services, behavioral health services, and community resources.

· Communicate abnormal and time-sensitive results promptly and document all actions and follow-up plans.

· Support transitions of care following emergency department visits, hospitalization, rehabilitation, or other healthcare encounters.

· Consult with or escalate to the supervising physician when a patient’s condition exceeds the PA’s scope, competency, or clinical privileges.

Documentation, Quality, and Compliance

· Complete accurate, timely, and legally compliant documentation in the electronic health record.

· Use appropriate diagnoses, procedure codes, and service levels based on the care actually provided.

· Comply with HIPAA, OSHA, infection-prevention, risk-management, patient-safety, and mandatory-reporting requirements.

· Participate in peer review, chart review, clinical meetings, quality-improvement activities, and population-health initiatives.

· Support quality measures involving chronic disease control, preventive screening, immunizations, care-gap closure, and follow-up care.

· Participate in credentialing, privileging, recredentialing, competency assessment, payer enrollment, and continuing education requirements.

· Perform other duties consistent with the position, approved privileges, and organizational needs.

Required Qualifications

· Master’s degree from an accredited Physician Assistant program.

· Current and unrestricted Texas Physician Assistant license.

· Current certification by the National Commission on Certification of Physician Assistants (NCCPA).

· Eligibility to practice and prescribe under applicable Texas physician supervision and delegation requirements.

· Current Basic Life Support (BLS) certification; ACLS or PALS may be required based on assigned duties.

· Successful completion of credentialing, privileging, primary-source verification, background screening, reference review, and other employment requirements.

· Demonstrated competence in primary care assessment, diagnosis, treatment, patient education, and clinical documentation.

· Effective communication skills and the ability to serve patients from diverse cultural, educational, and socioeconomic backgrounds.

Preferred Qualifications

· Two or more years of experience in family medicine, primary care, internal medicine, pediatrics, urgent care, community health, or a related outpatient setting.

· Experience in an FQHC, community health center, rural health clinic, public health clinic, or medically underserved setting.

· Experience treating both pediatric and adult patients and managing a broad range of acute and chronic conditions.

· Competency in minor office procedures and routine women’s health services.

· Experience in a high-volume outpatient practice and with electronic health records and population-health registries.

· Bilingual or multilingual ability relevant to the community served.

Physical and Scheduling Requirements

· Ability to perform the physical functions associated with outpatient clinical practice, including examining patients and using medical and computer equipment.

· Ability to sit, stand, and walk for extended periods and respond appropriately to urgent clinical situations.

· Availability to work assigned clinic hours, which may include occasional evenings, weekends, community events, or travel between approved locations.

Pay: $52.00 – $70.00 per hour
