Summary

We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our clinical team in an Inpatient Rehabilitation Facility (IRF) setting. In this role, you will conduct daily patient rounds, provide acute and transitional care management, and collaborate closely with physiatrists, therapy teams, and interdisciplinary staff to support optimal functional recovery and safe discharge planning for patients undergoing intensive rehabilitation.

Essential Duties & Responsibilities

  • Daily Rounding: Conduct structured daily rounds on assigned IRF patients, performing focused assessments, monitoring rehabilitation progress, and addressing acute and subacute medical needs.
  • Medical Management: Manage comorbidities and complications common in the IRF population, including post-stroke recovery, orthopedic post-op care, traumatic brain injury, cardiac conditions, and neurological disorders.
  • Interdisciplinary Collaboration: Partner with physiatrists, physical therapists, occupational therapists, speech-language pathologists, case managers, and nursing staff to develop and execute coordinated, patient-centered care plans.
  • Discharge Planning Support: Actively participate in discharge planning, including assessment of functional status, coordination with case management, and communication with receiving facilities or home health agencies.
  • Order Management: Write and manage orders for medications, diagnostics, therapies, and consults; respond promptly to nursing and therapy team communications throughout the day.
  • Urgent and Emergent Response: Evaluate and manage acute changes in patient condition, initiate appropriate interventions, and determine when escalation or transfer is warranted.
  • Documentation and Compliance: Maintain thorough, timely documentation in the EMR consistent with IRF regulatory standards, including required physician/APP attestation requirements and CMS compliance guidelines.
  • Patient and Family Communication: Communicate regularly with patients and families regarding medical status, goals of care, rehabilitation expectations, and discharge planning.
  • Quality and Outcomes: Participate in quality improvement initiatives focused on reducing hospital transfers, improving functional outcomes, and supporting IRF performance benchmarks.
