Full Job Description

Job Summary:
We are seeking a dedicated and experienced Nurse Practitioner (NP) to provide high-quality, patient-centered care in a post-acute setting. The NP will work collaboratively with physicians, nursing staff, and interdisciplinary teams to manage the medical needs of patients transitioning from hospital to skilled nursing, rehabilitation, or long-term care facilities. This role focuses on improving patient outcomes, reducing hospital readmissions, and ensuring continuity of care.

Key Responsibilities:

  • Perform comprehensive assessments, including history and physical exams for post-acute patients
  • Develop, implement, and manage individualized care plans
  • Diagnose and treat acute and chronic conditions
  • Order, interpret, and evaluate diagnostic tests and labs
  • Collaborate with physicians, nurses, therapists, and facility staff to coordinate care
  • Provide timely follow-ups and manage transitions of care
  • Educate patients and families on treatment plans, medications, and preventive care
  • Ensure accurate and timely documentation in compliance with regulatory standards
  • Participate in quality improvement initiatives aimed at reducing readmissions and improving outcomes

Qualifications:

  • Current, active Nurse Practitioner license in Texas
  • Board certification as a Nurse Practitioner (e.g., FNP, AGNP)
  • DEA license and prescriptive authority
  • Minimum [1–2+] years of experience in post-acute care, geriatrics, internal medicine, or related field preferred
  • Strong clinical decision-making and diagnostic skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively in a fast-paced environment

Preferred Skills:

  • Experience in skilled nursing facilities (SNF), long-term care, or rehabilitation settings
  • Familiarity with EMR systems
  • Knowledge of CMS guidelines and post-acute care regulations

Pay: $35.00 – $40.00 per hour
