Full Job Description

Overview
We are seeking skilled Wound Care providers to join our healthcare team. The ideal candidate will possess a strong background in patient care, with specialized knowledge in wound management and treatment. The Wound Care provider- PA or NP will work collaboratively with other healthcare professionals to develop and implement effective treatment plans tailored to individual patient needs. We are expanding in Ventura County, Los Angeles County, Santa Barbara County, and as far north as San Luis Obispo County, and are seeking experienced wound care professionals to join our team full time. This is a hybrid mobile and office role. You will travel to patients’ homes or facilities to provide wound assessments and treatments, document in our EMR, coordinate care, and contribute to quality initiatives.

Responsibilities

  • Assess and evaluate patients’ wounds, documenting findings accurately in medical records.
  • Develop and implement individualized wound care plans based on best practices and clinical guidelines.
  • Administer wound treatments, including dressing changes, debridement, and infection control measures.
  • Educate patients and their families on proper wound care techniques and self-management strategies.
  • Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
  • Participate in quality improvement initiatives related to wound care management.
  • Document visits and interventions accurately in accordance with practice standards.

Qualifications

  • Graduate of an approved school of nurse practitioner, or physician assistant program
  • Current, active California license (NP, or PA)
  • Wound care experience preferred but not required (home health, wound clinic, or hospital setting preferred)
  • Background in critical care, emergency medicine, or trauma medicine is advantageous.
  • Strong assessment skills with the ability to perform physical examinations and diagnostic evaluations effectively.
  • Completion of an accredited Wound, Ostomy, Continence Nursing (WOCN) program preferred but not required- willingness to obtain with practice support
  • Current CPR, health clearance
  • Strong patient education, communication, and documentation skills
  • Compassionate, patient-centered clinical approach

Pay: $125,000.00 – $145,000.00 per year
