Position Overview:
We are seeking a compassionate and motivated Physician Assistant to provide primary care, rounding services, chronic disease management, assessments, and relationship-based care throughout senior communities.
This role is ideal for candidates coming from primary care, hospital medicine, emergency medicine, internal medicine, geriatrics, urgent care, family practice, or post-acute settings looking for a more meaningful and patient-centered environment.
Key Responsibilities:
- Conduct patient assessments and follow-up visits.
- Provide ongoing primary and preventative care services.
- Manage chronic and acute medical conditions.
- Collaborate with nursing staff, caregivers, and interdisciplinary teams.
- Support transitions of care and continuity efforts.
- Document care plans and maintain compliance standards.
- Build strong relationships with patients and families.
Qualifications:
- Graduate of accredited Physician Assistant program.
- Active Minnesota PA license required.
- Strong communication and relationship-building skills.
- Experience in geriatrics, primary care, hospital medicine, or senior care preferred.
- Ability to work independently in a field-based setting.
