Nurse Practitioner NP or Physician Assistant PA for our Skilled Nursing Facility SNF

Compensation: $112,000 – $152,000 Annual Salary

Position Description for Nurse Practitioner NP or Physician Assistant PA:

We are looking for a dedicated and compassionate NP or PA for our Skilled Nursing Facility at Trinity Care Center in Farmington, MN for bedside patient care. The provider serves as the primary medical contact for assigned residents, partnering closely with the Medical Director, facility nursing leadership, and interdisciplinary teams. This is a full-time, in-person position Monday through Friday 9am to 5pm. No weekends, no nights, no on-call.

Qualifications for Nurse Practitioner NP or Physician Assistant PA:

  • Valid NP or PA license in the state of Minnesota, in good standing
  • Active DEA registration
  • 1+ years of clinical experience; SNF, geriatrics, or primary care preferred
  • Strong clinical documentation skills and familiarity with EMR systems
  • Excellent communication skills with patients, families, and interdisciplinary teams

Responsibilities for Nurse Practitioner NP or Physician Assistant PA:

  • Upon admission, examine residents and review their medical records to establish baseline assessments
  • Conduct routine follow-up visits and manage chronic conditions including diabetes, hypertension, CHF, COPD, and dementia
  • Prescribe and adjust medications including controlled substances per state regulations
  • Lead transitional care management and discharge planning with social work, nursing, and rehabilitation teams
  • Engage with residents’ families on care plans, goals of care, and advance directives
