We are seeking a dynamic and compassionate Adult Nurse Practitioner or Physician Assistant to join our Internal Medicine Clinic . In this vital role, you will provide comprehensive primary and specialty care to diverse patient populations, including adults, seniors, and individuals with complex medical needs. Your expertise will help improve patient outcomes through thorough assessments, evidence-based interventions, and collaborative care planning. Duties
- Conduct detailed patient assessments, including physical examinations and vital signs, to establish accurate diagnoses and treatment plans.
- Manage acute and chronic conditions such as geriatrics, internal medicine, emergency medicine, and behavioral health.
- Utilize EMR (Electronic Medical Records) systems like Elation for documentation, order entry, and patient data management.
- Administer medications and injections safely while providing patient education on medication management, lifestyle modifications, and preventive care.
- Coordinate care through case management strategies, discharge planning, utilization review, and collaboration with specialists in areas such as gastroenterology, infectious disease care, or hospice & palliative medicine.
- Participate in telehealth consultations
- Knowledge of CPT coding (Current Procedural Terminology), ICD-9/10 coding systems for accurate billing and documentation.
- Experience working with diverse populations including pediatric (pediatrics), neonatal (neonatal ventilator), adult (internal medicine), senior (geriatrics), memory care or dementia patients.
- Ability to perform triage assessments in urgent care or outpatient settings; experience managing infectious diseases or working within a Level II trauma center is advantageous.
- Strong understanding of HIPAA regulations to ensure patient confidentiality; capable of handling complex medical documentation including ICD coding and CPT procedures.
Pay: $225,000.00 – $255,000.00 per year
