- Nurse Practitioners (NPs)
- Physician Assistants (PAs)
- Medical Doctors (MDs)
- Doctors of Osteopathic Medicine (DOs)
- Qualified full-time Paramedics (select markets)

About the Role
Our mobile providers deliver comprehensive primary care services directly to seniors in:
- Private residences
- Assisted living facilities
- Independent living communities
- Other residential care settings
This care model helps improve continuity of care while reducing transportation and access barriers for older adults.
Providers work independently in the field while receiving support from centralized scheduling, remote medical assistants, and administrative teams.
Typical Responsibilities
- Conduct wellness visits and routine assessments
- Manage chronic medical conditions
- Review and manage medications
- Coordinate care with specialists, caregivers, and facilities
- Support preventative healthcare initiatives
- Document patient encounters accurately and timely
- Provide compassionate, patient-centered care in home-based settings
Preferred Experience
Experience in one or more of the following areas is strongly preferred:
- Geriatrics
- Primary care
- Home health
- Chronic disease management
- Palliative care
- Hospice care
- Urgent care
- Wound care
Qualifications
NP Candidates
- Active Michigan NP license
- MSN or DNP with board certification
PA Candidates
- Active Michigan PA license
- NCCPA certification preferred
MD / DO Candidates
- Active Michigan physician license
Paramedic Candidates
- Active Michigan Paramedic license/certification
- Full-time field availability preferred for select territories
What to Expect
The typical service area extends approximately 45 miles from your residence, with occasional travel beyond that range depending on operational needs.
While this is not an immediate opening for all markets, qualified applicants may be contacted as hiring needs become active.

Pay: From $110,000.00 per year
