Job Summary:
We are seeking a compassionate and skilled Physician Assistant or Nurse Practitioner to join our team. The ideal candidate will have expertise in wound care but is not required; on the job training will be provided as necessary. As a key member of our wound care team, you will play a vital role in assessing, treating, and managing complex wounds, while collaborating closely with physicians, nurses, and other healthcare professionals.
Location & Schedule
- Part Time, 16-20 hours per week
- Primarily office-based, including the following office locations; Fairfield, Trumbull, Shelton and Orange with possible rounding at local acute rehab facilities
Essential Responsibilities:
- Conduct comprehensive wound assessments and develop individualized treatment plans based on evidence-based guidelines and best practices
- Provide wound care procedures/treatments autonomously including debridement, I&D, wound culture, etc.
- Utilize advanced wound care modalities, such as negative pressure wound therapy, bioengineered skin substitutes, to optimize wound healing outcomes.
- Educate patients and caregivers on proper techniques, prevention strategies and the importance of adherence to treatment plans
- Utilize electronic health record systems for documentation
- Collaborate with multidisciplinary teams to ensure optimal patient outcomes
- Maintain accurate and thorough documentation of patient assessments, interventions, and outcomes in accordance with regulatory requirements and organizational policies
- Demonstrates organization, flexibility, and cooperation in performing job responsibilities
- Prescribe medications, including narcotics, controlled substances or class scheduled drugs with Supervising Physician’s documentation in the patient medical record
- Performs other duties as assigned by their supervisor
Education & Experience Requirements:
- Valid Nurse Practitioner or PA-C license in state of Connecticut
- Valid Controlled substance and DEA license
- 2+ years of wound care experience preferred
