Nurse Practitioner / Physician Assistant – Post-Acute Care
Southeast Superior Health is seeking a Nurse Practitioner or Physician Assistant to provide care in skilled nursing and assisted living facilities in the Jacksonville area.
This position offers a flexible weekday schedule, strong physician support, and a high level of autonomy within a growing regional medical group.
Position Details
- 5 days weekly
- Weekday rounding

Compensation
Expected compensation ranges from approximately $105,000–$140,000 annually equivalent, depending on schedule commitment, and productivity.
What We’re Looking For
- Active Florida NP or PA license
- Strong communication and documentation skills
- Ability to work independently and efficiently
- SNF, assisted living, geriatrics, hospital medicine, or primary care experience preferred but not required
