Full Job Description
We are a growing, physician-owned medical group expanding in McHenry, IL, seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a Skilled Nursing Facility (SNF) setting. This role focuses on post-acute and rehabilitative care, including orthopedic recovery, stroke management, and pain management, working alongside a collaborative lead provider. This role is weekends only!
Key Responsibilities:
- Provide comprehensive medical care to SNF patients, including assessments, diagnosis, and treatment planning
- Manage patients with orthopedic rehab needs, post-stroke care, and chronic pain conditions
- Ensure accurate and timely documentation in EMR
Required Qualifications:
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Illinois
- Active DEA license (or ability to obtain prior to start)
- Minimum 1 year of clinical experience as an APP (SNF, geriatrics, internal medicine, or rehab experience strongly preferred)
- Experience managing adult/geriatric patient populations
- Strong clinical decision-making and time management skills
- Comfortable working independently in an SNF setting
Preferred Experience:
- Skilled Nursing Facility (SNF) or post-acute care experience
- Background in internal medicine or geriatrics
- Experience with orthopedic, stroke, or pain management patients
Schedule & Setting:
- Part-Time, Weekends only: Saturday and Sunday
- No nights

Physical Requirements:
- Prolonged periods of standing, walking, and sitting while working with patients as well as working on a computer.
- Must be able to lift up to 50 pounds.
- May be exposed to bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
- Must wear the required PPE per company and/or facility requirements.
- Must comply with facility vaccination requirements.

Pay: $45,000.00 – $80,000.00 per year
