Full Job Description
Reports to the Manager or designee. Responsible for follow-up and care coordination of patients with lung nodules found through various avenues including, but not limited to high-risk lung screenings, and incidental findings. This role involves working closely with a multidisciplinary team, including pulmonologists, radiologists, primary care providers and surgeons, to ensure timely evaluation and diagnosis of patients to achieve optimal patient outcomes.

Coordinates care by:
- Coordinating interdisciplinary care conferences to review new or concerning lung nodules.
- Manages lung nodule patient database and follows up to verify next steps are being followed.
- Actively engage patients who have missed scheduled follow-up imaging.
- Assist in the development and implementation of protocols and guidelines for the management of lung nodules.
- Reporting information regarding variance trends and outcomes to key stakeholders and/or Physician reviewer for review and action.
- Prepare reports and presentations on clinic performance metrics.
- Participate in quality improvement initiatives as well as community outreach in the promotion of lung cancer screening and lung cancer awareness/prevention.
Participate in continuous quality improvement by:
- Compiling and analyzing data that is required for generating reports; also ensuring the data accurately represents utilization of trends and patterns.
- Preparing summaries and reports for review by stakeholders as needed/requested.
Coordinates the activities of the lung nodule clinics by:
- Collecting and reporting on newly found lung nodules across Beacon via Low Dose CT, incidental findings or referred to Lung Nodule Clinic
- Tracking progress in lung nodule patient database and verifying recommended follow up steps are completed.
- Acting as resource for providers/team of the lung nodule clinic and patient liaison.
- Coordinating the scheduling of necessary procedures and follow up required.
Performs other functions to maintain personal competence and contribute to the overall effectiveness of the department by:
- Completing other job-related assignments and special projects as directed.

- Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.

- Adheres to regulatory agency requirements, survey process and compliance.
- Complies with established organization and department policies.
- Available to work overtime in addition to working additional or other shifts and schedules when required.

Education and Experience
- The knowledge, skills and abilities as indicated below are normally acquired through the successful completion of an accredited nursing program. A current license to practice as an RN in Indiana is required. A minimum of three years of experience in oncology, pulmonary medicine, or a related field is preferred. Experience in care coordination or case management is preferred.
Knowledge & Skills

- Requires knowledge of counseling techniques and the ability to identify and utilize appropriate resources.

- Demonstrates sensitivity and the ability to use tact when working with patients and their families.
- Requires time management and problem-solving skills and the ability to independently prioritize and organize work activities; also requires the ability to work effectively under pressure.

Working Conditions
- Works in various environments with frequent changes in job demands.
- Requires travel to referral sources, satellite and other locations.
- Work hours will fluctuate based upon the needs of the patients served.
