Solomon Medical is seeking a full-time Behavioral Health Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing post-acute care team serving skilled nursing facilities in Central and Northern California.

This is a unique opportunity to practice behavioral health in a setting where your work is highly impactful, collaborative and relationship-driven. You will care for post-acute rehab patients and long-term care residents with psychiatric, behavioral, and cognitive health needs, while working closely with facility teams and physician leadership to improve quality of life and continuity of care.

Position Overview

This is a full-time onsite clinical role, five days per week, approximately 40 hours per week. The Behavioral Health APP will provide direct patient care in skilled nursing facilities and collaborate with the Behavioral Health Medical Director and facility teams to evaluate, diagnose, treat, and monitor patients with behavioral health needs.

The ideal candidate is compassionate, clinically strong, organized, and committed to improving patient care in the post-acute and long-term care setting. This role is well suited for a clinician who values autonomy, team-based care, and meaningful patient relationships.

The APP’s duties and responsibilities may include, without limitation, the following:

General Practice Responsibilities

  • Adhere to Solomon Medical’s policies, procedures, guidelines, and standards of care, and provide patient care in accordance with all applicable laws, regulations, and ethical standards.
  • Remain available by phone or Doximity when offsite.
  • Complete timely and accurate documentation sufficient to support Medicare-compliant medical decision-making and appropriate billing.
  • In accordance with Medicare guidelines and Solomon Medical’s standard of care, complete and close medical notes within twenty-four (24) hours to maintain accurate records and support high-quality patient care.

Behavioral Health Responsibilities

  • Conduct behavioral health evaluations for all newly referred patients, including completion of PHQ-9 screenings and any other clinically indicated psychiatric assessments.
  • Perform psychiatric evaluations using trauma-informed care principles, including assessment of mood disorders, anxiety, psychosis, cognitive impairment, substance use, behavioral disturbances, and safety risk.
  • Provide supportive psychotherapy during follow-up encounters when clinically indicated, including coping skills support, behavioral interventions, adjustment-related counseling, and crisis support.
  • Prescribe and monitor psychotropic medications in accordance with applicable laws, regulations, facility requirements, and CMS F605 requirements, including documentation of target symptoms and clinical rationale.
  • Participate in Gradual Dose Reduction (GDR) meetings in accordance with each facility’s requirements and collaborate with interdisciplinary teams regarding psychotropic medication management, including documenting clinical justification when GDR is contraindicated.
  • Ensure that required informed consent documentation for psychotropic medications is completed and maintained in accordance with applicable regulatory standards.
  • Collaborate with nursing staff and social services to ensure that psychiatric diagnoses, target behaviors, and non-pharmacologic interventions are accurately reflected in resident care plans.
  • Complete timely and accurate documentation sufficient to support Medicare-compliant medical decision-making and appropriate billing.

Qualifications

  • Active California Nurse Practitioner or Physician Assistant license in good standing
  • Prior behavioral health, psychiatry, geriatric psychiatry, post-acute, skilled nursing, or long-term care experience preferred
  • Experience conducting psychiatric evaluations and managing psychotropic medications strongly preferred
  • Strong communication and interdisciplinary collaboration skills
  • Ability to work independently within a physician-led team model
  • Commitment to high-quality documentation, compliance, and patient-centered care

Pay: $150,000.00 – $170,000.00 per year
