Position Summary:
The Nocturnal Advanced Practice Provider (APP) provides comprehensive overnight coverage for house-wide inpatient surgical and subspecialty services. This role supports cardiac surgery, general surgery, and additional surgical subspecialties by delivering high-quality, patient-centered care, performing invasive procedures, and ensuring continuity of care during nocturnal hours. The APP functions with a high degree of autonomy while collaborating closely with attending surgeons, fellows, residents, and multidisciplinary teams.
Essential Duties and Responsibilities:
Clinical Care–
- Provide overnight cross-coverage for inpatient surgical and subspecialty services.
- Perform comprehensive assessments, including history and physical exams, for new admissions, consults, and acute changes in patient condition.
- Diagnose and manage postoperative and acute surgical complications.
- Initiate and adjust treatment plans in collaboration with supervising physicians.
Procedural Competence–
- Perform invasive bedside procedures including, but not limited to:
- Central venous catheter placement
- Arterial line insertion
- Chest tube placement and management
- Thoracentesis and paracentesis
- Wound exploration and bedside surgical interventions
Specialty Coverage-
- Provide dedicated support for cardiac surgery patients, including:
- Postoperative hemodynamic monitoring
- Management of chest tubes, pacing wires, and anticoagulation
- Support general surgery and subspecialties (e.g., vascular, spine, gyn-onc) with acute care needs and general postoperative management.
Care Coordination-
- Ensure effective handoffs between day and night teams.
- Communicate changes in patient status to attending surgeons and relevant team members.
- Collaborate with nursing staff, respiratory therapists, and other disciplines to optimize patient outcomes.
Documentation & Compliance–
- Maintain accurate, timely, and thorough documentation in the electronic medical record.
- Adhere to institutional protocols, clinical guidelines, and regulatory standards.
Required Qualifications–
- Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program.
- Active, unrestricted state licensure as an NP or PA.
- Board certification (ACNP, AGACNP, or PA-C preferred).
- Minimum of 2–3 years of experience in surgical, critical care, or cardiothoracic settings.
- Demonstrated proficiency in invasive bedside procedures.
- Current BLS and ACLS certifications.
Preferred Qualifications–
- Experience in cardiac surgery or cardiothoracic ICU.
- Familiarity with hemodynamic monitoring and ventilator management.
- Experience with high-acuity, fast-paced inpatient environment

Work Schedule-
- Night shift position (e.g., 7:00 PM – 7:00 AM)
