Full Job Description
Hours: M-F either 5x8s or 4x10s to include one Saturday per month
Responsibilities:
- Provides ongoing high quality primary care to clients
- Works collaboratively with team members providing medical support and consultation as necessary for around 18-21 patients per day
- Performing comprehensive annual physicals, sick visits, and required follow ups
- Documentation of medical information in EMR system (eCW)
- Referrals to specialists as needed
- Maintains and documents the problem oriented medical record accurately after each client visit and monitors health maintenance checks of patient panel.
- Performs clinical procedures competently as necessary
Requirements:
- At least one full year of recent primary care experience
- Family Nurse Practitioner (FNP) or Physicial Assistant (PA) Licensure in the state of CT
- Graduate of an accredited Master’s program as either an NP or PA
- Current American Heart Association BLS/AED Certification
- Infection Control Certification
Pay: $4,000.00 – $4,200.00 per week
