Primary Care Provider (NP or PA) – Geriatric MedicineFull-Time | MedWell Medical | Long Beach & Orange County, CACompensation: $132,000–$170,000 per year (Base + Bonus Potential)

Position Overview
We are seeking a full-time Nurse Practitioner or Physician Assistant to join our growing team. In this role, you will manage a panel of approximately 100 patients and provide in-person primary care services directly at senior living communities. This is an ideal position for a provider who values meaningful patient relationships, autonomy, and impactful work in geriatrics.
Schedule
- Monday to Friday (Flexible hours)
- Optional nights, weekends, or on-call shifts for additional pay
Key Responsibilities
- Provide comprehensive care to a panel of approximately 100 senior patients
- Conduct physical exams and manage both chronic and acute conditions
- Order and interpret labs, imaging, and other diagnostic tests
- Develop and manage personalized treatment plans
- Educate patients and families on care and wellness strategies
- Collaborate with interdisciplinary teams and facility staff
- Participate in community outreach and education
- Accurately document visits using ICD-10 and HCC coding in the EHR
- Engage in end-of-life and palliative care discussions as needed
Qualifications
- Active California license as an NP, PA, MD, or DO
- Minimum of 1 year experience in geriatrics, internal medicine, or primary care (preferred)
- Strong skills in clinical judgment, patient communication, and care coordination
- Experience with palliative care and telehealth platforms is a plus
- Valid CA driver’s license and access to reliable transportation
Compensation & Benefits
- Base Salary: $132,000 – $170,000/year
- Total Compensation: $170,000+/year with bonus potential
