Duties
- Conduct thorough patient assessments, including physical examinations, vital signs, and medical history reviews to determine diagnoses and treatment plans.
- Manage acute and chronic conditions across various specialties such as geriatrics, pediatrics, behavioral health, and infectious disease care.
- Administer medications
- Utilize electronic health record (EHR) systems such as eClinicalWorks to document patient encounters accurately and efficiently.
- Provide health coaching and education tailored to individual needs, emphasizing preventive care and lifestyle modifications.
- Coordinate care with specialists for complex cases involving hospital medicine, ICU or PICU experience, dialysis management, or post-acute care planning.
- Participate in telehealth services to extend access to care in remote or underserved areas while adhering to HIPAA regulations for patient privacy.
Qualifications
- Current licensure as a Nurse Practitioner or Physician Assistant with eligibility to practice in the state of employment.
- Proven experience working in primary care settings within FQHCs or community clinics preferred.
- Demonstrated proficiency with EMR/EHR systems such as eClinicalWorks
- Strong clinical skills including knowledge of CPT coding, ICD-10/ICD-9 coding standards, and medical documentation best practices.
- Experience performing procedures such as suturing, phlebotomy, venipuncture, catheter care, sterile processing techniques, and basic life support (BLS).
- Familiarity with diverse medical areas including geriatrics, pediatrics , behavioral health, pain management, hospice & palliative medicine, occupational health, and infectious disease care.
- Ability to work effectively in high-pressure environments
- Excellent patient assessment skills with a solid understanding of physiology and anatomy; capable of triage and case management for complex cases.
