Patient Choice Medical Care is dedicated to delivering high-quality, patient-centered healthcare services. We are looking for a full time APRN or PA to see patients primarily in the office in Hamden.
Salary range will be supplemented by a bons structure.
Practice is independent, primary care private practice . Great support clinically as well as administratively.
Responsibilities
- Conduct thorough patient assessments and develop individualized care plans.
- Perform diagnostic tests and interpret results to guide treatment decisions.
- Educate patients and families on health management and preventive care strategies.
- Collaborate with interdisciplinary teams to optimize patient care and discharge planning.
- Provide support for home care services as needed, ensuring continuity of care.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Stay updated on best practices in nursing and healthcare trends.
Qualifications
- Valid PA license in the state of practice
- Proficiency in basic life support techniques and emergency response protocols.
- Excellent communication skills for patient education and team collaboration.

Pay: $110,000.00 – $135,000.00 per year
