Position Overview
Avant Wellness is seeking a Functional Medicine–Aligned Prescribing Provider with Illinois license to support integrative medication management across our patient population.
This is a part-time role with the opportunity to grow into full-time, based on mutual fit and patient volume.
Flexible structure:
- In-person (Chicago), hybrid
- 1099 Independent Contractor
Clinical Scope
The physician will support integrative prescription management for patients engaged in our care model. This provider thoughtfully bridges conventional and functional medicine utilizing prescriptions when appropriate and actively deprescribing when clinically indicated. This provider understands and is passionate about root-cause resolution, patient safety, and strategic prescription optimization within a holistic care model.
Examples include but are not necessarily limited to:
- Bioidentical Hormone Therapy (estrogen, progesterone, testosterone)
- Thyroid optimization (T4/T3 and combination approaches)
- Low-Dose Naltrexone (LDN)
- Other targeted medications aligned with functional medicine care
All prescribing and deprescribing is integrated alongside:
- Nutrition and lifestyle interventions
- Targeted nutrient therapy
- Herbal medicine
Responsibilities
- Evaluate and manage prescription therapies within a functional medicine framework
- Collaborate with:
- Nutritionists
- Non-prescribing physicians
- Review labs and contribute to care plan development
- Adjust medications based on patient response and data
- Document clearly and efficiently within structured workflows

The Functional Medicine Prescribing Provider will not be expected to carry the full burden of patient care alone. Instead, they will function as a medical partner within a structured, team-based system.
Ideal Candidate
We are specifically seeking someone who:
- Is an MD, DO, or NP (with full practice authority or appropriate autonomy)
- Has experience in or strong alignment with:
- Functional / integrative medicine
- Hormone therapy (BHRT)
- Thyroid and metabolic care
- Is collaborative, flexible, and team-oriented
- Is comfortable practicing within a structured care model
- Values patient education and long-term outcomes over quick fixes and symptom management
