Position Summary
Union Family Health Center is seeking a dedicated and compassionate Advanced Practice Provider (Physician Assistant or Nurse Practitioner) to serve as a primary care provider for our Health Center patients. This provider will deliver high-quality, on-site medical and nursing services to eligible health plan participants and their dependents. The role requires strong clinical judgment, a patient-centered approach, and the ability to work collaboratively within a multidisciplinary team.
Key Responsibilities
- Provide full-scope primary care services including assessment, diagnosis, treatment, and follow-up for acute and chronic conditions.
- Maintain and document thorough, accurate medical records; complete chart documentation and locking notes within 48 hours.
- Ensure 24/7 call coverage availability and timely responses to patient messages and voicemails.
- Prescribe medications and controlled substances in accordance with state and federal regulations, maintaining a current DEA and CDS number.
- Refer patients to specialists as appropriate and coordinate continuity of care.
- Adhere to all HIPAA regulations and applicable federal and state health data privacy laws.
- Actively support the Health Center in fulfilling obligations to its patient population.
- Maintain appropriate decorum and professionalism in all interactions with patients, staff, and partner organizations.
- Report any unfavorable occurrences, adverse events, or claims to management promptly.
- Maintain training, education, and skills commensurate with current medical standards and community practices.
- Provide supervision for all delegated care activities at the Health Center.
- Complete all required employment paperwork, credentialing, onboarding, and background checks.
Required Qualifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Current, unrestricted license to practice in the State of Nevada (and other applicable states).
- Valid and current DEA number and CDS number with good standing to prescribe controlled substances.
- Current National Provider Identifier (NPI).
- Successful completion of background investigation and credentialing processes.
- DOT Certification through NRCME (if occupational health services are part of the role).
- Must remain insurable at reasonable rates under the employer’s professional liability program.
- Not sanctioned, suspended, or excluded from any federal or state healthcare program (OIG, Medicare, Medicaid, TRICARE).
Schedule & Work Environment
This is a full-time position (1.0 FTE) with a schedule established by the employer. Hours may include early morning, evening, or weekend shifts depending on Health Center needs. The provider may occasionally be assigned to other Health Center locations within 100 miles of the primary work site, with five business days advance notice.
