Position Overview
We are seeking a compassionate and flexible Nurse Practitioner (NP) or Physician Assistant (PA) to join our team on a per diem basis. The provider will conduct in-home visits, delivering comprehensive medical care and improving patient outcomes through personalized treatment plans under direct supervision of the practice physician.
Responsibilities
- Conduct in-home medical visits, including assessments, diagnoses, and treatment plans
- Manage acute and chronic conditions
- Perform physical exams and order/interpret diagnostic tests
- Prescribe medications and therapies as appropriate
- Provide patient and caregiver education
- Maintain accurate and timely documentation in EMR
- Administer IV fluids/vitamins infusion
- Perform blood-draws when necessary
Qualifications
- Active and unrestricted NP or PA license in NY
- Board certification (NP: FNP, AGNP, etc. / PA-C required)
- Minimum of [1–2] years of clinical experience preferred
- Experience in home care, geriatrics, or primary care is a plus
- Strong clinical assessment and decision-making skills
- Excellent communication and interpersonal skills
- Reliable transportation and valid driver’s license
